Support Planning Services
Mind Kind’s Support Planning Services are designed to empower individuals by helping them take an active role in shaping their care. Through a Person-Centered Planning approach, we work closely with each participant to identify their goals, needs, and preferences. Our Support Planners assist with developing a comprehensive Plan of Service (POS) that reflects the individual’s desired outcomes and coordinates all necessary Medicaid Long-Term Services and Supports (LTSS). We also help applicants navigate eligibility requirements, understand their options, select qualified providers, and access resources that promote independence, dignity, and quality of life.
Case Management Services
Our Case Management Services focus on ongoing coordination, advocacy, and monitoring to ensure participants receive the right services at the right time. Mind Kind Case Managers serve as trusted partners, regularly checking in with participants, tracking service delivery, identifying potential barriers, and resolving issues that may affect progress. We collaborate with providers, families, and community resources to ensure services are effective, person-centered, and compliant with Medicaid standards. Whether transitioning from institutional care or maintaining stability at home, our Case Managers are committed to helping participants reach their full potential and live confidently in their communities.

Benefits & Participant Rights
Individuals enrolled in CPAS, CFC, HCBOW, or ICS programs are entitled to important benefits and protections designed to uphold their dignity, independence, and well-being. At Mind Kind, we ensure these rights are respected and built into every aspect of our service delivery.
Person-Centered Planning
Participants have the right to direct their own care—choosing providers and making decisions about how services are delivered based on their preferences and goals.
Dignity & Respect
All services must honor each participant’s values, culture, identity, and personal preferences.
Fair Hearing & Appeal Rights
If a service is denied, reduced, or changed, participants have the right to appeal and request a fair hearing to review the decision.
Freedom of Choice
Participants may select services from any willing and qualified Medicaid provider without restriction or pressure.
Confidentiality
Personal and medical information is handled with the highest standards of privacy and shared only with authorized individuals.
Quality of Care
Every participant is entitled to receive safe, effective, and high-quality services that meet state and federal Medicaid standards.
