Support Planning Agency
Mind Kind’s Support Planning Agency helps individuals take an active role in making decisions about their care and the life they want to live. Using a person-centered approach, our Support Planners work closely with each participant to understand their strengths, needs, goals, and preferences. We guide participants through the Medicaid Long-Term Services and Supports process, assist with developing and monitoring their Plan of Service, explain available options, support the selection of qualified providers, and connect them with helpful community resources. Throughout the process, we advocate for each participant’s voice, freedom of choice, independence, dignity, and overall quality of life.
Case Management Agency
Mind Kind’s Case Management Agency provides person-centered support to individuals receiving Medical Day Care services. Our Case Managers work closely with participants, families, representatives, providers, and community partners to understand each person’s needs, goals, and preferences. We help participants navigate the Medicaid Long-Term Services and Supports system, develop and monitor their Enrollment Packet and Plan of Service, coordinate services, maintain eligibility, and address barriers that may affect their care. We also protect each participant’s right to make informed choices and advocate for services that promote safety, dignity, independence, and quality of life.

Benefits & Participant Rights
Individuals enrolled in CPAS, CFC, HCBOW, ICS or MDCSW programs are entitled to important benefits and protections designed to uphold their dignity, independence, and well-being. At Mind Kind, we ensure these rights are respected and built into every aspect of our service delivery.
Person-Centered Planning
Participants have the right to direct their own care—choosing providers and making decisions about how services are delivered based on their preferences and goals.
Dignity & Respect
All services must honor each participant’s values, culture, identity, and personal preferences.
Fair Hearing & Appeal Rights
If a service is denied, reduced, or changed, participants have the right to appeal and request a fair hearing to review the decision.
Freedom of Choice
Participants may select services from any willing and qualified Medicaid provider without restriction or pressure.
Confidentiality
Personal and medical information is handled with the highest standards of privacy and shared only with authorized individuals.
Quality of Care
Every participant is entitled to receive safe, effective, and high-quality services that meet state and federal Medicaid standards.

Guide you through eligibility and enrollment in the appropriate Medicaid LTSS program
Develop and update a Plan of Service (POS) tailored to your unique goals and needs
Coordinate care, monitor service delivery, and address any challenges that arise

Provide continuous support and reassess your services as your needs change
