Support Planner Supervisor
The Support Planner Supervisor oversees and managesport planners to ensure high-quality, person-centered planning and case management services are provided to program participants. This role includes staff supervision, training, performance evaluation, and ensuring compliance with program policies, procedures, and state and federal regulations. Support Planners
The Support Planner provides comprehensive case management and support planning services to individuals with complex medical and/or behavioral health needs, older adults, adults with disabilities, and youth with disabilities. The role requires developing person-centered plans, coordinating services, and ensuring participants receive appropriate and timely support to achieve their goals of living independently in their communities. Credible Messenger
The Credible Messenger serves as a mentor, advocate, and role model to at-risk individuals, particularly youth or justice-involved individuals, using their lived experiences to build trust, foster positive relationships, and inspire personal and community transformation. This role involves providing participants guidance, support, and accountability while collaborating with community organizations and stakeholders to create pathways to success.
