Program Eligibility and Service Offerings
Overview of CPAS, CFC, HCBOW, and ICS Programs
Mind Kind provides support planning and case management services for individuals enrolled in Maryland’s Medicaid Long-Term Services and Supports (LTSS) programs. The four primary programs include:
1. Community Personal Assistance Services (CPAS)
Purpose: Provides personal care assistance to individuals who need help with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) to remain in their homes and communities.
Key Services:
In-home personal assistance
Help with bathing, dressing, meal preparation, and medication management
Nurse monitoring visits
Assistance with scheduling medical appointments
Eligibility Criteria:
Must be enrolled in Maryland Medicaid
Must require assistance with at least one ADL or IADL
Services must be determined to be medically necessary
2. Community First Choice (CFC)
Purpose: Offers personal assistance services, home modifications, and support to help individuals live independently rather than in an institutional setting.
Key Services:
Personal assistance (similar to CPAS)
Assistive technology and medical equipment
Home accessibility modifications (ramps, grab bars, etc.)
Personal emergency response systems (PERS)
Consumer training and family caregiver support
Eligibility Criteria:
Must be enrolled in Maryland Medicaid
Must require assistance with ADLs or IADLs
Must meet nursing facility level of care criteria
3. Home and Community-Based Options Waiver (HCBOW)
Purpose: Helps individuals transition from nursing homes to the community and provides long-term services to prevent institutionalization.
Key Services:
All services available under CFC and CPAS
Assisted living services
Medical daycare
Behavioral health consultation
Family caregiver training and support
Moving assistance for nursing home transitions
Eligibility Criteria:
Must be age 18+ with a disability or age 65+
Must meet nursing home level of care criteria
Must meet Medicaid financial eligibility requirements
Must be at risk of institutionalization without support
4. Increased Community Services (ICS)
Purpose: Helps individuals in a nursing facility transition into the community with long-term support.
Key Services:
Personal assistance services
Home modifications
Assistive technology
Community transition support
Housing assistance
Eligibility Criteria:
Must currently reside in a nursing facility for at least 90 consecutive days
Must require nursing home level of care
Must be financially eligible for Medicaid
A safe community housing option must be available. Verification and Service Access
Mind Kind assists participants in verifying eligibility and accessing services within these programs.
Eligibility Verification Process:
Initial Screening:
Review participant financial, medical, and functional eligibility
Determine which program best fits their needs
Application and Enrollment:
Assist with completing Medicaid LTSS applications
Submit required medical documentation and eligibility forms
Assessment and Service Planning:
Collaborate with medical professionals to complete required assessments
Develop a Plan of Service (POS) based on participant needs
Service Coordination:
Identify and connect participants with approved service providers
Monitor services and ensure ongoing eligibility compliance
Benefits and Participant Rights
All individuals enrolled in CPAS, CFC, HCBOW, or ICS are entitled to the following benefits and rights:
✅ Person-Centered Planning: Participants direct their care, selecting providers and determining service preferences.
✅ Freedom of Choice: Participants choose from any willing Medicaid provider without restrictions.
✅ Dignity & Respect: All services must be provided in a manner that respects participant rights, culture, and preferences.
✅ Confidentiality: Personal and medical information must be kept private and only shared with authorized parties.
✅ Fair Hearing & Appeal Rights: If services are denied or reduced, participants have the right to appeal and request a fair hearing.
✅ Quality of Care: Participants are entitled to safe, effective, high-quality services that meet state and federal standards.
Mind Kind’s Role in Supporting Participants
🔹Guide participants through the eligibility and enrollment process
🔹 Develop and update the Plan of Service (POS) based on participant goals
🔹 Coordinate care, monitor services, and address barriers to service access
🔹 Advocate for participant rights, ensuring conflict-free case management
🔹 Provide ongoing support and reassess service needs regularly
